Both Federal and State law require Illinois State University to periodically report information about the University’s workforce and equal opportunity efforts.

To assist in reporting, the Office of Equal Opportunity, Ethics, and Access requests all faculty and staff complete the Voluntary Self-Identification Survey by logging in to myIllinoisState.edu and navigating to the survey in the Announcements Section. We ask that all employees, even those who completed the survey previously, complete this new survey as some of the information we are required to report has changed.

Completion of this survey is strictly voluntary. Disclosure or refusal to provide information will not result in adverse treatment. Responses will be kept confidential and all information will be used only in a manner that is consistent with the University’s Federal and State reporting requirements. Responses will not be used to determine eligibility for other University programs and/or processes which may require self-identification.
